President Morgan called the meeting to order at 5:15 p.m.

Leah Brink, newly appointed City Ex-Officio, was welcomed. Brink gave a brief introduction of herself to the group.

INVITED GUEST
Merkley welcomed and introduced Karen Weber, Brookings Health System’s Inpatient Care and Emergency Department Director. Weber has been a valuable employee for over 34 years and is deeply involved in mental and behavioral healthcare throughout her role at Brookings Health System and within the community. Weber has served on numerous boards within the Brookings community and currently attends East Central Behavioral Health stakeholder meetings.

Weber presented an overview of how mental illness impacts us locally and how Brookings Health System has partnered with other agencies to provide the best possible care. Statistics from 2017 and 2018 were shared on age range and number of individuals presenting in the Emergency Department with a primary chief complaint related to a behavioral health emergency, i.e. suicidal threat or attempt, followed by severe mental illness and acute psychosis (violent outbursts seem to be more prevalent in these individuals). Year over year, there has been a steady incline in the number of mental illness emergency services with a 39% increase in involuntary commitments between 2017 and 2018.

The role of Brookings Health System is to provide necessary medical treatment while collaborating with other agencies such as East Central Behavioral Health, Human Services Center in Yankton and Avera Behavioral Health who evaluate and help guide the individual to further treatment and transition. From a financial perspective, reimbursement is very low for BHS. Payor sources include self-pay, Medicaid/Medicare, Private Insurance and Brookings County. In 2018, total charges incurred were $328k with reimbursement of only $75k.

Staff training is an important component in managing a mental health situation. Management of Aggressive Behavior (MOAB) training teaches participants how to protect themselves from injury, and at the same time, control individuals without causing them harm. Law enforcement is heavily involved in crisis situations as mode of arrival to the Emergency Department is often by police officers. The Crisis Intervention Team (CIT) Program trains officers to identify severe mental illness, de-escalate the situation, and facilitate assessment/treatment in a safe location (mobile response or transport to ED). We now have a state CIT Coordinator and funds to provide on-going training to law enforcement and EMS.

East Central Behavioral Health recently hired Mary Beth Fishback as the Director and they are in the process of expanding staff to support patients in Brookings County.

Weber was thanked and recognized for being instrumental in leadership, guidance and continued collaboration on mental and behavioral healthcare. Weber commented, continued collaboration and communication between all agencies is critical and she is honored to be involved with other city and county leaders to positively impact the mental health and wellbeing of our community.

Krogman thanked Brookings Health System for adding the Safe Room to the Emergency Department which is the protocol for rooming high security patients and for having trained staff to care for individuals in a crisis situation.

Merkley noted a new case mix model; the Patient Driven Payment Model (PDPM) will be implemented in October, 2019 for skilled nursing facilities. PDPM is designed to be budget neutral, decreases therapy reimbursement and rewards nursing care. It is predicted The Neighborhoods at Brookview will achieve neutral to positive revenue under the new payment model. Administrator, Jason Hanssen, will be invited to a board meeting this fall to highlight some of the major changes with the new payment model.

CFO Report
Wagner provided further explanation on the Net Days in Accounts Receivable ratio. Progress is being made to bring Net Days in AR down which will result in an increase in revenue. Wagner also stated the transfer of outstanding accounts to one collection agency has been completed.

CNO Report
Hillestad commented on the presentation by Karen Weber in regards to training staff in Management of Aggressive Behavior (MOAB). Brookings Health System’s Quality Director is certified to teach the program and most recently provided training to our business office staff that could potentially be the first point of contact and encounter aggressive behavior from a patient/visitor. Hillestad also noted several departments have held Department of Health mock surveys as part of the hospital’s Survey Readiness Program.

Board Committee Reports
Administrative Committee:
Administrative Committee member Wosje provided a report; the Administrative Committee met on June 18, 2019. Topics of discussion included Bartley Optical acquisition, 2nd Floor Medical Plaza, CEO Employment Contract, Elkton Clinic, Days Cash on Hand Strategy, Mental/Behavioral Health, Orthopedic Service Line and ENT Service Line. There were no action items.

Building Committee:
Morgan provided a report; the Building Committee met on June 18, 2019. Topics of discussion included 22nd Avenue Reconstruction, 2nd Floor Medical Plaza, Parking Lots. Members also toured the Yorkshire Plaza Building and Home Health’s new location. There were no action items.

Finance Committee:
Finance Committee Chair Peterson provided a report; the Finance Committee met on June 19, 2019. The committee reviewed and approved the payables, adjustments, and financials for April and May. Other topics included an update on the Collection Process and Cash Investment/Strategy.

Quality Committee:
Quality Committee Chair Ellsworth provided a report; the Quality Committee met on June 18,, 2019. Home Health Director, Lynne Kaufmann, presented information on Home Health & Hospice quality reporting and patient experience survey results. Other topics included an influenza vaccination summary and iVantage Top 20 Rural Hospital Results. There were no action items.

Dr. Ellsworth noted Brookings Health System had an overall ranking of 98.6 on the Hospital Strength Pillar Composite Report which ranks BHS at 11th in the nation. Additionally, our highest score fell in the Quality pillar. This is impressive in comparison to the other 19 rural hospitals included in the Top 20 in the nation.

OLD BUSINESS
CEO Employment Agreement
The Administrative Committee will gather sample language from other hospitals and the City of Brookings regarding the severance portion of the agreement. A review of the information will be completed at the committee level for consideration.

NEW BUSINESS
Medical Staff Credentialing
A motion was made by Ellsworth, seconded by Peterson to approve the credentialing and re-credentialing from April, May and June as recommended by the Medical Executive Committee.

Action: All present voted yes; motion carried.

Q1 2019 Hospital/NBV Performance Dashboards
Merkley reviewed the overall pillars of success (Finance, Growth, People, Quality, and Service) performance snapshot for the Hospital and The Neighborhoods at Brookview. Q1 2019 reflects a successful start to the year.

Brookings Economic Development Corporation
Merkley noted a request from BEDC was recently received requesting the health system’s continued support and commitment for 2020. Brookings Health System has been a key partner for several years with an annual investment of $50k.

Brookings Health System Educational Scholarship
Merkley stated this year’s recipient of the $1k BHS Educational Scholarship is Matthew Osmanski. Matt will be attending Augustana University enrolling in pre-med courses. Matt is the son of Michelle Osmanski who is employed at Brookings Health System.

Brookings Healthcare Auxiliary
Merkley informed members the Brookings Healthcare Auxiliary group will be disbanding as an official organization on July 1. Several other auxiliary groups across the state have disbanded, including the State of South Dakota Auxiliary Association. Our group of former auxilians will continue to meet as a social club and do many of the same activities for the benefit of the health system and its staff. They have chosen to rename their group as the Brookings Health System JOY (Just Older Youth) Club.

20th Street South interstate interchange
Merkley stated Brookings Health System has been asked to provide a letter of support for the BUILD grant application for the 20th Street South interchange project. The interchange will provide better access to the industrial park as well as address safety concerns with current traffic congestion. After discussion all members were in agreement.

A motion was made by Wosje, seconded by Anderson to provide a letter of support to accompany the BUILD grant application for the 20th Street South interchange project.

Action: All present voted yes; motion carried.

MED Magazine
Dr. Ellsworth was thanked for representing Brookings Health System in the most recent Midwest Medical Edition (MED) magazine. Ellsworth stated he was honored to represent Brookings Health System and provide his point of view on why BHS was chosen as one of the Top 20 Rural Community Hospitals.



Upon motion by Ellsworth, second by Wosje the meeting adjourned at 7:03 p.m.
